Our nurse practitioner model provides exceptional preventative and primary care services for patients
throughout their lives. Our fully staffed office is conveniently located in Brooklyn,
Ohio, which serves as a base for our interdisciplinary practice.
We strive to coordinate care by working closely with our medical directors, specialized physicians, and
other health care providers. Further, our primary care providers travel off-site including to hospitals, clinics, and patient homes, to accommodate a variety of patients in the most comfortable setting for them.
We provide primary care and preventative health services, treat acute and chronic conditions, prescribe medications, and order and interpret diagnostic tests. We aim to lower the cost of healthcare by reducing emergency room visits and offering lower medication costs.

Our team is dedicated to bridging the widening gap between doctors and patients by providing affordable services on location. This means fewer doctors’ visits and less complicated care, all while providing the same results.

Our group works closely with a seasoned Medical Director who works with our Nurse Practioners to provide quality preventative and primary care services for patients of all ages. These services include routine physicals, management of acute and chronic illnesses, coordination of care, medication management, and preventative health treatment. This also includes referrals to qualified specialists, home care, and hospice services.
Our office is conveniently located in Brooklyn, Ohio, close to the freeway. We are available to see patients in a variety of settings in the Greater Cleveland Area. Our practice accepts most insurance and is currently accepting new patients with same day appointments often available.

Lake Effect Health specializes in on-site wound care. The American Board of Wound Management has endorsed Seana as a Certified Wound Specialist since 2015. As an expert in the field, Seana diagnoses, treats, and manages both acute and chronic wounds utilizing the most up-to-date treatment guidelines and coordinating care with other health professionals.
